A series of failings have been identified in the lead-up to the horrific killing of June Fox-Roberts, a much-loved great-grandmother, in her own home in Pontypridd. Luke Deeley, a 25-year-old University of South Wales student, entered Mrs. Fox-Roberts’ home in November 2021 and brutally attacked her with an axe in what authorities described as a random and senseless act. Deeley later pleaded guilty to manslaughter, revealing that he was under the care of adult mental health services at the time, as he struggled with paranoid schizophrenia.
Three separate reports outlined missed opportunities in Deeley’s care, emphasizing poor communication between various agencies involved in his life, including the health board, council, police, and the university.
