A man was found dead just days after being released from prison two months early. Steven Hatch, aged 46, had requested a lower opioid prescription while in jail to tackle his addiction. However, shortly after leaving prison, he overdosed on a lethal mix of drugs. The prisons ombudsman’s report revealed that Mr. Hatch had been sentenced to eight months in September 2022 for burglary. Despite the typical practice of serving half the sentence in jail, Mr. Hatch was released in late November under a home detention curfew and tragically found dead on December 6.
Upon arriving in prison, Mr. Hatch informed a nurse that he was not currently using drugs but later tested positive for substances like buprenorphine and benzodiazepine. He disclosed a history of drug addiction and was prescribed methadone to treat opioid dependency. Despite expressing a desire to reduce this dose, he was released with a 30ml prescription. Post-release, he requested to switch to Buvidal for monthly treatment but failed to engage with medical appointments.
Ultimately, Mr. Hatch’s life ended from mixed drug toxicity, including heroin, methadone, pregabalin, and an illegal sedative. The ombudsman’s report did not identify faults in the care provided to Mr. Hatch whilst in prison. He had been warned about drug risks and supported in his treatment plan, which included consultation with community services post-release.
Despite the tragic outcome, Mr. Hatch’s case highlighted the challenges individuals face transitioning from prison to society, especially in managing substance misuse issues. The need for comprehensive aftercare and ongoing support post-release to prevent relapse and overdose incidents like this remains a crucial focus for prison and healthcare services.
